Work Permit Application Form
- Student, parent and employer may sign the work permit below, and drop it off in the Counseling Department at the College & Career Center.
- During the summer, you can get your work permit issued at the Petaluma City Schools District Office: 200 Douglas Street, Petaluma CA 94952
- Please keep in mind, work permits will be issued within 24-48hrs during school hours (Monday-Friday 8AM-3PM).
Letter of Recommendation (LOR) Profile
- Fill out the form with as much details as possible.
- Kindly email your recommenders (teacher, counselor, etc) and use professional etiquette.
- Give your recommenders at least 2-3 weeks to write your LOR.
