Transcripts may be requested by submitting the online Transcript Request form linked below. By submitting this request electronically, you certify that you are the current/former student and that the information provided on and in connection with this form is true, accurate and complete.

To request an OFFICIAL or UNOFFICIAL copy of your CGHS transcript:

• Please allow 48-72 hours for your request to be processed.
