How It Works

  1. Student completes the online form. (Student has already completed and submitted an application for admission.)
  2. Form is automatically forwarded to the parent email entered on the form.
  3. After Parent approval, the form is automatically forwarded to the School Approver at the high school (based on the high school selected or entered on the form.)
  4. After the School Approval is entered, the form is automatically forwarded to the Admissions & Records Office for processing.
  5. The Parent and the School Approver will receive an email notification and a copy of the submitted form.

The form can be easily completed and submitted from desktop or tablet computers, and is easily completed with a smart phone as well.

What If There’s a Problem with a Submitted Form?

For troubleshooting issues related to submitting the online High School Dual Enrollment Request Form, email: admissions@santarosa.edu. Please include the name of the high school, name of the student, and the student’s SID (if available). Please note that the online form does not lift enrollment blocks instantaneously. Therefore, please permit time for the form to be routed for signatures and A&R processing.
