Work Permit Application Form

Student, parent and employer may sign the work permit below, and drop it off in the Counseling Department at the College & Career Center.
During the summer, you can get your work permit issued at the Petaluma City Schools District Office: 200 Douglas Street, Petaluma CA 94952
Please keep in mind, work permits will be issued within 24-48hrs during school hours (Monday-Friday 8AM-3PM).

Letter of Recommendation (LOR) Profile

Fill out the form with as much details as possible.
Kindly email your recommenders (teacher, counselor, etc) and use professional etiquette.
Give your recommenders at least 2-3 weeks to write your LOR.
