Medication Authorization Form

This form is to be filled out by the student's physician or health care provider to allow the Health Office staff to administer medication during the school day. Contact the school nurse if you have questions. Forms for medication administration are available at your child’s school.

Data Confirmation happens at the beginning of each school year through Aeries. If you would like paper versions of the forms for data confirmation, please contact your school to inquire further.

Petaluma City Schools is pleased to announce a new online facility request and rental system designed to provide a positive user experience for all. With the new system, reservation requests can be submitted easily and will be handled more efficiently.
