Forms & Required Documents

Welcome to Petaluma City Schools! In order to enroll and maintain attendance within our school District, certain forms and documents are required. While required documents can vary, below you will find a file of forms and documents you may need in addition to your registration form/application. Please consult with your school on which documents you need to fill out.

Required Documents


During the registration process, you will need the following documents in order to complete your student's registration:

  1. Date of birth confirmation such as a copy of original/certified copy of birth certificate, passport, or baptismal certificate
  2. Proof of address such as a utility bill, rental agreement, or property tax payment receipts
  3. Immunization Records (Your child will NOT be able to start school until you have provided documentation of ALL required immunizations)
    1. Shots Required for K-12
    2. Requisitos de Vacunación (K-12)

Inter-district Transfer Form (English/Spanish) or Intra-district Transfer Form (2025-262026-27), if applicable.
